What to include in the T&C document
1. Acceptance of Services
By booking an appointment with Hydration Concierge Mobile IV Therapy, LLC (“the Company”), you acknowledge that you have read, understood, and agree to these Terms & Conditions, as well as our Consent Forms, Privacy Policy, and No-Refund Policy.
2. Medical Oversight & Eligibility
All services require a telehealth consultation by a licensed medical provider prior to treatment.
The Company reserves the right to decline service if you do not meet medical eligibility or if it is determined that IV hydration or vitamin therapy is not appropriate for your condition.
3. Client Responsibilities
Clients agree to:

Provide accurate medical history and disclose all medications, allergies, and conditions.

Follow all pre- and post-care instructions.

Notify the nurse immediately if any symptoms or concerns arise during or after treatment.

Ensure the treatment environment is safe, clean, and pet-free during service.
4. Payment & Fees
All services must be paid in full at the time of booking.

After-hours or holiday services may incur additional charges.

Tips are optional and never required.
5. No Refund Policy
All sales are final.
Once a service is booked or products are purchased, no refunds will be issued.
If you do not complete the consultation or are deemed medically ineligible, you may reschedule but not receive a refund.

6. Cancellation & Rescheduling
Cancellations less than 24 hours before your appointment are subject to the full-service charge.

No-shows will be charged the full booking amount.

Clients may reschedule one time without penalty if done 24+ hours in advance.
7. Service Limitations
Hydration Concierge Mobile IV Therapy does not replace emergency care or primary medical treatment.
We do not diagnose, treat chronic conditions, or prescribe medication outside the scope of hydration and vitamin therapy.
Call 911 for any medical emergencies.
8. Risks & Consent

The Company makes no guarantees regarding specific health outcomes, weight loss, recovery times, or performance improvements.
10. Privacy & Confidentiality
All client information is protected in accordance with HIPAA standards.
We do not share or sell personal information.
11. Right to Refuse Service
The Company reserves the right to refuse service to anyone for safety, medical, or behavioral reasons including intoxication, aggressive behavior, unsanitary environments, or failure to comply with terms.
12. Liability Waiver
By receiving services, clients agree to release and hold harmless Hydration Concierge Mobile IV Therapy, LLC, its nurses, contractors, medical directors, and affiliates from any liability related to:
